At-Home Urine Test Program Terms and Conditions

The At-Home Urine Test Program (the Program) will provide eligible patients with one at-home Minuteful Kidney test at no cost to the patient, subject to these Terms and Conditions. The complimentary test is sponsored by Boehringer Ingelheim. The Program is intended to help educate patients about chronic kidney disease and associated cardiovascular risk and provide them an opportunity to take a complimentary screening test at home that is an early step in learning more about their kidney health.

The Minuteful Kidney Test is a prescription, home-use urine test for the semi-quantitative measurement of albumin and creatinine levels in urine. The Minuteful Kidney test is manufactured and distributed by Healthy.io. It is not a Boehringer Ingelheim product. The Minuteful Kidney test is intended for use by individuals at risk of kidney disease. Any questions or concerns about the Minuteful Kidney test should be directed to Healthy.io and/or your doctor. Boehringer Ingelheim is not providing medical advice or entering into a physician–patient relationship with Program participants. The Program does not replace independent medical advice or judgment regarding diagnosis or treatment.

The Minuteful Kidney test does not enable diagnosis of kidney disease or associated cardiovascular risk. Your doctor may use the results, together with a clinical evaluation and additional testing, to help assess your kidney health. The Minuteful Kidney test is not intended to substitute consultation with your doctor regarding the diagnosis or management of kidney disease or any other condition. The test cannot detect every instance of kidney disease, and an abnormal test result does not necessarily indicate that a patient has kidney damage. An abnormal result may be indicative of kidney disease or a variety of other conditions and may be affected by a number of factors, including, but not limited to, dehydration, diet, and sample timing.

Talk to your doctor if you have questions regarding whether you are an appropriate candidate for this test or regarding your test results and for any questions about your health.

By participating, you attest:

  • that you meet the Program Eligibility Criteria;
  • if eligible, that you are responsible for determining whether participation in this program is right for you;
  • that you accept and agree to comply with these Terms and Conditions; and
  • that you agree to release Boehringer Ingelheim, to the fullest extent permitted by law, from any liability/losses (without limitation) relating in any way to the Minuteful Kidney test and/or your participation in the Program.

Program Eligibility Criteria: Patients must reside in the U.S. in states where the Program is available, be at least 18 years of age, have been diagnosed by their healthcare provider with type 2 diabetes mellitus and/or hypertension, have not been diagnosed with chronic kidney disease, and have not have taken a uACR (urine albumin-creatinine ratio) test in the past twelve (12) months.

One test per patient for lifetime. While supplies last. Boehringer Ingelheim may change the terms of the Program at any time, including discontinuing it.

Patients, providers, and other third parties may not seek or obtain reimbursement for this test from any health insurer or other third party, including state and federally funded programs.

Void where prohibited or restricted by law. In AK, AL, AZ, CO, DC, DE, GA, IL, KS, MD, MI, MT, ND, NJ, NM, NV, NY, and WV, the program is not available. Other restrictions may apply.

The Program does not obligate use of any specific medicine or require you to follow up with any particular healthcare provider.

The test is intended solely for patient benefit, not for healthcare providers. The test may not be sold, purchased, transferred, traded or offered for sale to anyone.
